NOTICE TO TROY INTERNAL MEDICINE PATIENTS:
Effective October 1, 2008, we will be
implementing a new policy regarding prior authorizations for prescription drugs.
The pre-authorization process imposed upon the physicians and staff by your
insurance company has forced us to make this new policy. We spend a great deal
of time reviewing your chart and discussing this with your insurance company’s
prior authorization department. To meet the demands while continuing to provide
good patient care, prior authorizations for prescription drugs will be handled
as follow:
| | 1. | All prior authorizations will be directed to our Prior Authorization
Department |
| 2. | If you, your pharmacy, or your insurance company have prior
authorization forms, please fax them to 248-267-6791 with the necessary
information already completed. |
| 3. | No prior authorizations will be done at the
time of an office visit. |
| 4. | A service charge of $10.00 will be applied to each
prior authorization, not to exceed $30.00. |
Please understand the majority of the
prescriptions and refills do not require prior authorizations and will continue
to be processed as we have always done in the past. We apologize for any
inconvenience this may cause, but we believe this will help streamline the prior
authorization process that has been implemented by many of your insurance
programs, and better serve your health needs
